Obverse: Profile bust of Pius IX looking to proper right, wearing cap and lowered hood with wreath at edge; inscribed at edge “PIVS IX·PONT·MAX·” and under bust “NIC·CERBARA F·”.

Reverse: Monument within an apse; inscribed bove monument “ALEXANDER·VII·PM/LABA[ ]M·APSIDEM/REPA[ ]T·ORNAVIT/AN[ ] M·DC·LXIII”, and at edge “[flower] PIVS·IX·P·M·BASILICAE·LATERAN·ALTARE·MAX·AD·VETEREM·FORMAM·RESTIT·AC·SPLENDIDIORI·CVLTV·INSTAVR·”.
