Studio Art Class Student Form

The Studio Art Class Student Form is required for students under the age of 18 and enrolled in a Studio Art Class at the Museum of Fine Arts, Boston. Please complete this information prior to the start of classes. Records must be updated annually and filled out completely.

When minors are left in our care for in-person classes, it is important that we have access to the student’s emergency contacts, health information, allergies and authorizations in the event of an emergency in order to relay relevant information to medical professionals.

MFA Studio Art Classes are committed to students of all abilities having a positive experience. Please indicate if the student needs any of the following accessibility features.
In order to support a successful experience for all students in the program, please select any additional accommodations that would be beneficial. MFA staff will do their best to provide an inclusive environment. Understand that interventions may not always prevent challenging behaviors. Please review the MFA’s Behavior Policy.

*Personal care attendants are welcome to accompany the student for the duration of class and are admitted free of charge.

Authorizations

Students age 12 and above may dismiss themselves at the end of classes. Please confirm that the student has permission to self-dismiss.
Hand sanitizer is used periodically, in addition to soap and warm water, to clean students' hands while attending the program.
The emergency and medical information listed above is correct and accurately reflects the health status of the individual to whom it pertains. The person described has permission to participate in all MFA Studio Art Class activities except as noted above and/or by an examining licensed medical professional. If myself or the emergency contacts listed above cannot be reached in an emergency, I give my permission to the MFA Department of Protective Services to contact the Boston Emergency Medical Services to hospitalize, secure proper treatment for, and order injection, anesthesia, or surgery for the individual. I understand the information on this form will be shared on a "need to know" basis with MFA staff and that additional health information and immunization records may be requested.
I hereby give my consent for the Museum of Fine Arts, Boston ('MFA') to photograph, film, videotape and/or record the student's voice, likeness and/or artwork to use, reproduce and distribute in any media for the purpose of promoting programs presented by the MFA, and other educational purposes. I understand the resulting photo, film, videotape, or recording shall be the sole property of the MFA. I furthermore acknowledge that the MFA is under no obligation to use or publish this material. I understand and agree that I will receive no fee or payment for the student and/or their artwork being photographed, filmed, videotaped, or recorded. I hereby grant the MFA a worldwide, royalty-free, perpetual license to the student's image, name, voice, likeness and/or artwork for all standard museum purposes including, but not limited to, promotional materials for the MFA and its programs and exhibitions, publications, educational programs, archival purposes, and public relations purposes.
